  • Job Summary:

    The Lead Post Release Worker will be responsible for supporting the Assistant Program Director of Post Release in the day to day operation of the Post Release department including assigning of case loads, review of post release reports and internal audits of department staff. The Lead Post Release Worker will also maintain a post release caseload. All duties will be performed based on administrative training and direct supervision of the Assistant Program Director or Program Director. The Lead Post Release Worker will maintain a flexible, organized and efficient work schedule and is subject to work overtime and be on-call. 

    Essential Functions:

    • Assign caseloads to post release workers.
    • Perform monthly audits of program case files, complete quarterly sponsor surveys and complete biyearly staff shadowings to ensure compliance with SWK and federal guidelines.
    • Develop corrective action plans or disciplinary memos to address performance issues of post release worker staff in collaboration with Assitant Program Director of Post Release and Program Director.
    • Review all post release reports before submission to any stakeholder, ensuring that all concerns have been addressed
    • Maintain a daily updated list of the status for all cases to ensure that adequate progress has been made and timelines are being upheld
    • Prepare and submit weekly case logs reports to local funding source representative and other stakeholders as approved by Program Director
    • Carry a small caselod of post release cases
    • Review client documents provided by shelter caseworker
    • Conduct initial post release visit to establish goals and complete treatment plan
    • Conduct subsequent post release visits to monitor progress of goals for reqired timeframe.
    • Complete post relase visit documentation in a timely, professional manner;
    • Link client and family with community resources and services;
    • Participate in phone conferences with team members;
    • Maintain confidentiality for all client and program operations;
    • Review and maintain appropriate documentation;
    • Establish helping relationships with client and families

    Other Functions:

    • Attend all organizational required training
    • Attend all departmental and program meetings to ensure that up-to-date information is received and/or information of policy changes or practice are adhered t
    • Travel as needed for home visits, trainings and conference.
    • Maintain a safe work environment that is free from distractions
    • Develop and maintain productive relations with local, state, and federal contacts.
    • Able to react to change productively and handle other essential tasks as assigned.

    Qualifications and Requirements:

    • Master’s degree in social work, with (2) years direct service delivery experience with family reunification or family preservation and/or foster care and adoption home study preparation, may include part-time, volunteer or internship experience. OR;
    • Bachelor’s degree in social work with five (5) years direct service delivery experience and family reunification or family preservation and/or foster care and adoption home study preparation, may include part-time, volunteer or internship experience.
    • Any educational or experiential qualifications will be considered as equivalent, in lieu of the above minimums and will require prior approval from the Regional Executive Director or designee. 
    • Must possess excellent organizational, writing, analytical, and processing skills. 
    • Should be able to perform conflict resolution or crisis intervention and have excellent communication skills. 
    • Must possess excellent time management skills and be flexible to schedule changes.
    • Must be computer literate with working knowledge of Microsoft Windows (Word Perfect, Microsoft Word, Excel).
    • Must possess a valid state driver’s license.
    • Must be available to travel.
    • Cleared Tuberculosis test results.
    • Cleared background check from appropriate entity.
    • Bilingual. (English/Spanish)
    • Cleared drug test results (this one is for Texas programs only).
    • Must be at least 21 years of age at the time of hire.

    Physical Demands:

    Job requires extended periods of sitting; use of various office equipment; traveling by car and plane for extended periods of time.

    Work Environment:

    This is a work from home job where high levels of self-motivation and organizational skills are needed. The Lead Post Release Worker will work closely with staff in various time zones throughout the United States therefore early morning or after hour work may be needed. Availability to travel will also be needed.
