Job Summary:

 

The Case Aide is directly assigned to the home study and post release program and is responsible for assisting in the day to day tasks to ensure that services are provided to youth in a timely manner. The Case Aides are required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours, weekends, and be on-call.

Essential Functions:

  • Must ensure that youth are properly enrolled in the program’s internal intake system (ETO, UAC portal).
  • Audit ETO and UAC portal to ensure compliance and accuracy.
  • Assist with accepting referrals, managing active case logs, and contacting sponsors and families as directed by Regional Lead or designee.
  • Maintain data on all clients enrolled in the program and all dismissed clients.
  • Contact youth and sponsors as directed by Regional Lead or designee.
  • Document all actions taken and contacts with youth, sponsor, and stakeholders in the form of progress notes (touchpoints) as required by SWK, State and Federal contracts.
  • Adhere to budgetary limits as approved by Program Director.
  • Assist with the coordination of travel arrangements for program staff for purpose of completing home visits (may include booking airfare, hotel, and rental car).
  • Assist program staff with preparing home visit documentation including locating case specific resources/services prior to a scheduled home visit.
  • Assist with completing sponsor surveys as directed by Regional Lead or designee.
  • Assist with filing, closing, and with overall daily maintenance of records found in either the physical case file and on ETO or UAC Portal software.
  • Assist with making referrals to TVAP and OTIP.
  • Assist with managing and updating resource spreadsheet.

Other Functions:

  • Attend all organizational required trainings and facilitate trainings as needed.
  • Attend all departmental and program meetings to ensure that up-to-date information is received and/or provided and to ensure that changes to policy or practice are implemented and adhered to.
  • Assist with the submission of monthly SWK reports, funder’s quarterly reports, and end of year reports.
  • Provide and facilitate information as needed for SWK and/or funders monitoring activities.
  • Travel as needed for trainings, conferences or other program needs.
  • Attend training that will enhance professional growth in the area of home study and post release and documentation or other topics as deemed appropriate by the Program Director.
  • Maintain a safe work environment that is free from distractions.
  • Able to react to change productively and handle other essential tasks as assigned.

Qualifications and Requirements:

  • Minimum GED or High School Diploma.
  • Must be computer literate with working knowledge of Microsoft Windows (Word Perfect, Microsoft Word, Excel).
  • Required to work a flexible schedule to facilitate program services.
  • Cleared Tuberculosis test results.
  • Cleared background check from appropriate entity.
  • Bilingual (English/Spanish)
  • Must be at least 21 years of age at the time of hire.

 

Preferred:

  • Thirty (30) hours of higher education in a social service field.

-OR-

  • One to two (1-2) years of paid or unpaid experience working in a youth services field.

Physical Demands:

Job requires extended periods of sitting and computer work.

Work Environment:

This is a remote program where employees work from home and travel. Work shifts are subject to change based on program needs. Employees must have a quiet, private space in which to work at home.
