The Lead Medical Coordinator (LMC) is responsible for guiding and ensuring the quality of clinical services, treatment, and/or interventions provided to Unaccompanied Minor’s (UM) in an assigned SWK program in collaboration with the Regional RN and contracted physicians. The LMC is devoted to overseeing delivery of medical care in Southwest Key program settings with the emphasis on disease prevention and health maintenance.   The Shift Supervisor is responsible for the daily operations for all shifts ensuring that ratios are met at all times. The Shift Supervisor is subject to work overtime and be on call. The Shift Supervisor will maintain a flexible, organized and efficient work schedule.

  The Service Coordinator/ Group Facilitator is responsible for the delivery of direct services to the clients and families specifically facilitation of group and recreational sessions as designated by the Community Youth Development Program. The Group Facilitator maintains a flexible, organized and efficient work schedule and is subject to work more than a normal work day and be on-call to facilitate program services.

 The Clinical Supervisor is responsible for providing clinical and administrative supervision and oversight of the Multisystemic (MST) program and staff. The MST Clinical Supervisor must maintain a flexible work schedule that will include working hours other than regular work schedule and evenings, weekends, and holidays.  May carry a small caseload based on need of the program.  

The Community based therapist is responsible for providing Multi Systemic Therapy services per the model through ongoing support and training on the MST model. The MST Therapist must maintain a flexible work schedule that will include working primarily afternoons and evenings, some weekends and will be included in an on-call rotation.

  The Assistant Lead Medical Coordinator (ALMC) is responsible for guiding and ensuring the quality of clinical services, treatment, and/or interventions provided to Unaccompanied Minors (UM)  in collaboration with the Regional Director of Nursing and contracted physicians. The ALMC is devoted to the delivery of medical care in Southwest Key program settings with the emphasis on disease prevention and health maintenance. Additional responsibilities include ensuring compliance with State licensing, Office of Refugee Resettlement (ORR), and all policies and procedures implemented by Southwest Key Programs.    

  The Clinician is responsible for providing mental health screenings and counseling intervention services, as well as continually monitoring and documenting children’ mental well-being and overall adjustment to the Unaccompanied Children program. The Clinician’s functions are performed as part of a multi-disciplinary team and include development and implementation of minor counseling plans, facilitating referrals, transfers, and reunification planning. All duties will be performed based on education, training, and certified counseling experience within the social service and/or mental health fields. The clinician is subject to work extended hours and weekends, and to be on-call.

The Medical Coordinator is responsible for assessing, facilitating and monitoring all components of client medical services. Additional responsibilities include ensuring compliance with regulations in policy development and implementation according to State Licensing and ORR, as well as on-going program refinement based on advances in the program.

  The Youth Care Worker (YCW) is responsible for providing direct care supervision for youth residing in Southwest Key Programs (SWK) care fostering an environment that facilitates learning and behavioral management in a supportive manner. The Youth Care Worker will maintain a flexible, organized, and efficient work schedule.

  The Lead Case Manager is responsible for the day to day operations of the case management department including the timely submission of reunification packets, service plans and other assessments required for the compiling of a comprehensive case file found on Southwest Key’s (SWK) web-based system (ETO) with supporting documentation maintained in physical files. The Lead Case manager is required to maintain a flexible, organized and efficient work schedule and is subject to work extended hours, weekends, and be on-call.  #SWSR

  The Service Coordinator is responsible for coordinating the services for youth enrolled in the Community Youth Development program.   Program services are available to participants according to their social, community and school needs, which is disclosed upon intake. The Service Coordinators duties include the ability to facilitate program components on a weekly basis, specifically the YLD and Parent involvement components. Service Coordinator ensures youth have access to all program components. Program services include Youth Leadership Development Sessions, Tutoring, Mentoring, and Parents Groups. The Service Coordinator serves as a family liaison, community outreach worker, and group facilitator.

The Clinician provides home and community-based behavioral health services such as, assessment, triage, and treatment to individuals diagnosed with mental illnesses who may also have co-occurring substance use disorders and/or chronic health conditions.   The Post Release Worker II is responsible for providing Level 1 & 2 post release services to youth who have been released from ORR’s custody to a sponsor in the United States. The Post Release Worker II is responsible for assessing, facilitating and monitoring all assigned caseloads. The Post Release Worker II promotes the safety and well-being of children and coordinates supportive services in the community where the child resides. The Post Release Worker II reports to the Lead Post Release Worker or designee.
